What is Remote Therapeutic Monitoring (RTM)?
RTM stands for Remote Therapeutic Monitoring. It allows a care team to monitor certain aspects of your health and therapy progress remotely, without requiring you to go into an office for every check-in.
What health issues would make me a good candidate for RTM support?
You may be a good candidate for RTM if you have a diagnosed condition that benefits from regular monitoring, therapy follow-up, symptom tracking, medication adherence support, or extra care coordination at home.
Do I have to change my doctor or insurance plan?
No. You should not have to change your doctor or insurance plan just to see if you qualify. Your eligibility and coverage are reviewed during enrollment, and EverThrive or the assigned service provider can help confirm how your benefits apply.
Can I have RTM if I already have RPM or CCM?
Usually, you should not be enrolled in overlapping RTM, RPM, or CCM programs at the same time unless your provider or care team confirms it is allowed and appropriate. EverThrive's enrollment process asks you to confirm that you are not already enrolled in another RTM, RPM, or CCM program. If you are unsure, EverThrive can help you review your situation before enrollment.
How does RTM affect my daily life?
RTM is designed to fit into your normal routine. Depending on your care plan, you may answer simple check-in questions, use the EverThrive app, report how you are feeling, respond to reminders, connect with your Care Advocate, or engage with wellness content. EverThrive also adds daily engagement features such as messages, check-ins, games, inspirational quotes, photo sharing, educational videos, and reminders.
What does my Care Advocate do for me?
Your Care Advocate helps you stay connected, supported, and engaged. They may help you understand the program, complete check-ins, use the EverThrive tools, stay consistent with your care plan, and communicate important updates to the appropriate care team when needed. Your Care Advocate does not replace your doctor. They give you an extra layer of support between visits.
Do I need to be tech-savvy to benefit from RTM?
No. You do not need to be tech savvy. EverThrive is designed to be simple and supportive, and your Care Advocate can help you get comfortable with the process.
Do I need to go anywhere to benefit from RTM?
No. The point of RTM is to help your care team support you remotely between traditional appointments.
How would I qualify for RTM?
You may qualify if you have a condition that can benefit from remote monitoring, therapy follow-up, symptom tracking, or care support at home. After you enroll, a licensed healthcare provider reviews your information to confirm whether RTM is appropriate for you.
Do I have to travel anywhere if I get accepted into the RTM program?
No travel is required. EverThrive is built so you can receive support from home using a device you already know, such as a phone, tablet, computer, or TV.
Are there any costs?
Most participants pay nothing out of pocket. Standard Medicare cost-sharing may apply, but supplemental coverage, Medicaid, or Medicare Advantage may cover those costs. Your coverage should be reviewed before or during enrollment so you understand whether any cost applies to you.
What are the key benefits of RTM for me?
RTM through EverThrive gives you: more support from home (your care team can monitor important health information remotely and respond when something changes); fewer gaps between visits (RTM helps your care team understand how you are doing between appointments, not just when you are in the office); better follow-through with your care plan (tracking therapy adherence, therapy response, symptoms, medication adherence, and patient-reported outcomes); daily human connection (video chats, messages, check-ins, caregiver and family connection, and ongoing encouragement); and more confidence at home (the program is designed to help people age more confidently and independently at home).
What are the three categories of RTM and what conditions apply to each?
Respiratory System Monitoring covers breathing-related therapy response, respiratory symptoms, and respiratory therapy adherence — for conditions like asthma, COPD, bronchitis, and chronic respiratory conditions. Musculoskeletal System Monitoring covers pain, movement, therapy adherence, rehab progress, and physical therapy response — for arthritis, chronic joint pain, back pain, neck pain, post-surgical rehab, and chronic pain. Cognitive Behavioral Therapy Monitoring covers CBT-related therapeutic monitoring and digital therapeutic engagement when appropriate — for behavioral health support connected to provider-directed CBT care plans.
Can I cancel at any time and for any reason?
Yes. You can discontinue RTM services at any time and for any reason, without penalty.

They can help you stay engaged with your care plan, complete check-ins, use the platform, understand your daily wellness tools, and keep important care-related information moving to the right people when needed.

What is the difference between RTM, RPM, and CCM?
RPM (Remote Patient Monitoring) focuses on physiological data like blood pressure and weight. CCM (Chronic Care Management) coordinates ongoing care for complex conditions. RTM (Remote Therapeutic Monitoring) focuses on therapeutic adherence and behavioral, musculoskeletal, and respiratory conditions — including therapy response, symptom tracking, and patient-reported outcomes.
